Clinical Pharmacy
The UCSF Department of Clinical Pharmacy vision is “to bridge gaps in patient care, especially for the underserved. “ We are an organization of people—clinical pharmacists, researchers, administrators, scholars and staff—all focused always on ensuring patients benefit from the safe and effective use of medicines to treat, prevent, and cure disease. We care for patients, teach tomorrow’s pharmacists, mentor those who have already earned degrees, serve the public, and research important health care issues.As a critical component toward the realization of our vision, we have expanded our clinical services to establish the UCSF Medication Management Service in Fresno, California. This program represents the first department initiative managing chronic diseases in our underserved patients in the Central Valley. Our key to ultimately improving patient care and health has always been working with colleagues in the health professions, in industry and government, and constantly sharing our knowledge with scientists on one end of the spectrum to policy makers on the other. It is this approach which will allow us to bridge gaps in health care for our underserved Fresno-based patients. We look forward to partnering with you to provide high quality patient care in the Fresno community. B. Joseph Guglielmo, PharmD Thomas A. Oliver Professor and Chair in Clinical Pharmacy Clinical Pharmacy is an approach that was created at the School of Pharmacy, University of California, San Francisco (UCSF) and the UCSF Medical Center in the 1960s. In this practice model, pharmacists work as members of the health care team, share their drug expertise with physicians, nurse practitioners, nurses, and physician assistants, and work directly with patients to ensure patients get the best results from their medicines. Pharmacists are the drug experts on the health care team and provide their services directly to patients wherever they are needed. The UCSF Department of Clinical Pharmacy supports the UCSF Medication Management Service-Fresno. The service is an example of the department's vision of being the best at bridging gaps in patient care, especially for the underserved. The Service exemplifies two of the UCSF School of Pharmacy's strategic planning goals:
